Overview

This application allows you to build Online Forms to collect various data from your visitors. You can use one the form templates to create your form, but there is also a drag-and-drop feature for additional elements. You can add any of the following elements to your form:

  • Text
  • Heading
  • Text Box
  • Password field
  • Text Area
  • Drop-down
  • Check Box
  • Radio Button
  • Date
  • HTML
  • Image
  • Button


Create New Form


Create HTML forms in order to collect certain data from your Website visitors. You can choose one of the following template designs:

  • No Template- opens a blank page.
  • Blank Email - opens a page with an Email field and Submit button.
  • Contact Request - opens a page with First Name, Last Name, Country, State, ZIP code, Address, Email fields and a Submit button.
  • Feedback - opens a page with first Name, Last Name, Email and a multiline text box and a Submit button.
  • Support Request - opens a page with a Full Name, Email and multiline text box and a Submit button.

Creating an online form is accomplished through the Create New Form wizard. You can access the wizard form the Main Menu page. You can also start the wizard by selecting Add new Form from the Manage Forms page.


To Start the Create New Form Wizard
  • On the Main Menu page, click Create New Form.

  • On the Manage Forms page, click Add new Form.

  • click the New Form tab.
To Create a New Form

Choose template

  1. On the Online Forms page, select a template for your form.
  2. Click Next.

Design Form

  1. On the Create New Form page, click on a form element and drag it to the board.
  2. To edit the label, click on the added element.
  3. To edit the element properties, click Untitled1.JPG.
  4. To remove the element from the board, click Untitled15.JPG.
  5. To edit the form properties, click Edit Form.
  6. When your form is complete, clcik Next.
  7. On the Setup Complete page, you can:
    Create a new form
    Manage your existing forms

Manage Created Forms

The Manage Created Forms page allows you to change your from settings or publish, download or delete your forms.


To Edit Form Elements
  1. On the Main Menu, click Manage Created Forms.
  2. Click the Untitled1.JPG of the from you want to edit.
  3. When the edits are complete, click Save or Publish Form.
To Edit Form Settings
  1. On the Create New from page, click Edit Form.
  2. Enter a name for your form in the Title field.
  3. Choose a background color for your form.
  4. Choose font type using the drop-down menu.
  5. Choose font size.
  6. Choose font color.
  7. Enter a location where you would like the submissions to be redirected, once the form is submitted.
  8. Upon submitting the form the users will be displayed with the text you enter here.
  9. To be sent an email when a form is submitted, select Yes from the Send e-mail drop-down.
  10. Enter an email address where you will be receiving submissions.
  11. Enter an alternative email address in the CC Email field, if required.
  12. Click Apply.

• Click “close” to exit from this page.

Note: If you click Close without clicking Apply, your changes will not be saved.


To Publish your Form

 

  • Click on “Untitled16.JPG ” to publish your form.

To View your Form in HTML format

 

  • Click on “Untitled12.JPG ” in order to view your form.

To Delete a Form

 

  • Click on “ Untitled15.JPG” to delete a form your list of existing forms.



Results (for v1.0 only)

Choose the format and download the results for a form you have created and published.
